Bleach might leave a smell too, so try a solution of 1/3 water, 1/3 rubbing alcohol, and 1/3 vinnegar. Then put the top on.
Dump it out after a couple of weeks, and leave the bottle open to ventilate, until it is bone dry inside, which is likely to take a couple of weeks too.
Then fill 1/2 full of backing soda. Put the top back on and shake it every day 2x a day (mornin/nite) for a week. Pour out the soda, then rinse and dry well. No more smell.
...bleach.
